Insurance Review
We accept and bill insurance as a courtesy. Alternatively, you may choose to bill your own insurance (we can provide a superbill) or pay a Time-of-Service (TOS) fee, which includes your copay, coinsurance, or deductible as determined by your insurance plan. All TOS payments are due at the time of service. Our staff can provide information on current insurance and billing options. You are ultimately responsible for any remaining balance after insurance coverage, including unpaid services. The clinic will rebill for a date of service only once if an error was made by our office. Please note that we do not bill secondary insurance carriers at this time.
Supplemental Financial Details
1. Payment Methods: We accept cash, Visa, Mastercard, AMEX, and Discover.
2. Payment Timing: All payments are due at the time of service unless prior arrangements have been made.
3. Time-of-Service Fees: All TOS fees are due at the time of service. Once your insurance coverage has been verified, we will inform you of any remaining financial responsibility.
4. Insurance Billing: As a courtesy, we will bill your insurance company. If there is a discrepancy, we will inform you as soon as possible.
5. Credit Balances: If you have a credit balance, we will reimburse you within 90 days or apply a credit to your account after payment has been received.
6. Supplements and Supplies: All supplements and supplies must be paid for at the time they are received.
7. Timely Payments: You are responsible for timely payment (within 4 weeks) of your account after receiving your statement.
8. Interest on Outstanding Bills: Outstanding bills will accrue interest at 1.5% per month.
9. Collections: Unpaid bills over 180 days and after three written or emailed notices will be reported to collection agencies, and your card on file will be charged for the unpaid balance.
10. Insurance Claims: We bill the insurance company you have provided. We track your insurance balances during the calendar year. If your benefits become exhausted during your treatment, we will inform you promptly.
11. Workers' Compensation Claims: We will bill workers' compensation cases directly to the insurance company, provided the appropriate paperwork has been completed and a proper claim filed. If the claim is denied, any unpaid services will be billed at the cash rate in accordance with Oregon law.
12. Personal Injury/Motor Vehicle Accidents: We will bill personal injury and auto accident cases to your auto insurance company as provided.
13. Non-Covered Items: Generally, supplements, herbal formulas, and medical supplies are not covered by insurance and must be paid for at the time they are received.
